New Medicare Patients
Internal Medicine Associates is currently accepting new patients who have Medicare as their primary coverage. Interested patients should call our office at 847-462-5100 to obtain more information. We will schedule your first appointment with one of our providers as soon as we receive 2 years of medical records from your current Primary Care Physician. We thank you for your interest in joining our practice!
Insurance Plan Participation
We participate in all major commercial insurance plans, as well as select HMO plans contracted with the Advocate Physician Partners Physician-Hospital Organization (Advocate PHO).Drs. Murati and Weiss are currently accepting both new commercial and new HMO patients who belong to insurance plans affiliated with the Advocate PHO.
Insurance Verification
All patients are required to bring their insurance card with them to every office visit. Patients who do not have the required documentation may be required to pay-in- full at the time of service. In addition, laboratory tests, in-office services and referral authorization may be deferred until your current benefits can be verified.
Co-Pays/Claim Filing
All co-pays are due at the time of service. We accept cash, personal checks, Visa, MasterCard and Discover. We will file your claims for all services rendered at each office visit.
