Welcome to Internal Medicine Associates, S.C.! In order to prepare for your first visit, please review the following guidelines:
- Our office runs very much on time. Please arrive 15 minutes before your scheduled appointment to allow for registration and form completion.
- We require all patients to bring their current insurance card to each visit.
- We require a minimum of 24 hours advance notice for appointment cancellations. A $30 fee is assessed for appointments canceled or missed without the requested advance notice.
- We strongly recommend that you check with your insurer regarding "Wellness" coverage. Please review our "Wellness vs. Diagnostic" explanation.
- For safety and health reasons, children are not allowed in the examination rooms. We suggest that you make alternate arrangements for child care prior to your visit.
- All co-pays are due at the time of service. We accept cash, personal checks, Visa, MasterCard, and Discover. We do not accept American Express.
- We ask that you print out and complete or review the following forms prior to your visit.
- Please print out and complete the Patient Registration Form.
- Please print out and complete the Patient Medical Questionnaire.
- Please print out and complete the HIPAA Communication Profile.

- Please print out and sign the HIPAA Consent Form.
- Please print out and sign the Advance Beneficiary Notice for Wellness Coverage.
- In order to avoid delays in service or rescheduling, please be sure to bring all of your completed and signed forms with you to your first visit!
